Action Based Enterprises developed the Transition Program for students with developmentally disabilities and their families who are seeking an uninterrupted transition from high school to adult services. ABE staff participates with the school department and family to develop an IEP and a transition plan that clearly define evaluation data that will be required for future approval by adult agencies. When appropriate, a social worker from Rhode Island Department of Behavioral Healthcare, Developmental Disabilities and Hospitals (BHDDH), will also collaborate in the facilitation of the transition plan.
Students eligible for the program should be between 18 and 21 years of age.
Regularly Scheduled Activities
Clients meet together and plan activities for the day
Consistent with IEP, students are assigned academics in the area of math, reading and writing
Career exploration and vocational activities; generating interest in potential jobs
Clients will perform cooking, landscaping or housekeeping activities, usually on site
Activities of Daily Living
Clients demonstrate ability to perform self-help skills in the area of grooming, hygiene and health
Phys Ed/Leisure Activities
Time is set aside each day for clients to engage in recreational and/or health/fitness related activities
School departments are responsible for transporting clients. However, in some instances, ABE can provide transportation at a very reasonable rate.
