Community Based Supports
Community-Based Supports include a range of social, recreational, and vocational activities designed to foster independence and community integration for adults with ID/DD. Our person-centered approach recognizes the uniqueness of each individual.
Eligibility
In order to be eligible for services, individuals must be 18 years or older and diagnosed with an intellectual or developmental disability, or a severe, chronic disability.
Students can apply to the Division of Developmental Disabilities any time after the age of 16.
Planning
Once the application is approved, a State caseworker will be assigned and will make referrals to provider agencies, like ABE.
ABE, or another provider, will work with the individual to develop an Individualized Service Plan outlining goals and the types of services to be delivered.
Outcome
ABE will provide staff to deliver services, and support coordination to insure that progress is being made toward individual goals in the areas of health, safety, social skills, and employment.
The participant becomes more independent and fulfilled after participating in community-integrated social and vocational activities.
